With respect to the below items and manufacturing lots:

Keystone has identified a rare manufacturing defect that can potentially lead to a hazardous condition where the lamp driver/base can separate from the LED array, thereby posing a risk of the LED array portion of the lamp falling. The products were known to be sold from March 2016 through August 2017.
There have been no injuries reported to date.
Out of an abundance of caution, Keystone is issuing a voluntary recall for the above noted lamps with the date codes listed, when installed in the following conditions:
1) The lamps are installed in fixtures with the lamp base “UP”
AND
2) Open fixture, no lens or full enclosure below the lamp.
Given the intended replacement application for these lamps, Keystone feels this represents a small amount of the overall installed base for the product.
